About Us

HomeFront Health (HFH) is a service-driven, tech-enabled mobile healthcare organization that delivers frontline medical services to patients in their home as an extension of hospital- and provider-centric care models. By harnessing the untapped resource of military-trained and field-tested Veteran Medics, our mobile medical units provide unparalleled patient care when and where it’s needed the most.

We are dedicated to delivering steadfast, clinically-sound intervention and care-coordination on behalf of hospitals, payers, employers, risk-bearing organizations, and healthcare systems across the primary care spectrum.

HFH medics act as the ultimate interface between hospital and patient — allowing hospitals to proactively engage their community, maximize patient-provider contact time, and give patients the peace of mind and confidence to re-engage the healthcare system for the right needs at the right time.
